Pivot Pembrokeshire is a partnership between the British Red Cross, Royal Voluntary Service, Pembrokeshire Association of Community Transport Organisations and Care and Repair.
It offers support to vulnerable adults with the aim of preventing unscheduled admission to hospital and supports people at home on their discharge from hospital.
Support includes transport to home from hospital and out-of-hours home adaptions and home safety checks.
Where appropriate, it also offers ongoing support, including emotional and practical support, and referral to relevant organisations and agencies.
