Care in the home

We provide short-term care and support in the home for people after an accident or illness, giving them the confidence to continue their daily lives. This service can be provided following a stay in hospital or to prevent unnecessary hospital admissions.

The service is available to people over 65, who live in the Omagh area but are in hospital anywhere in Northern Ireland. Patients are referred to the service by the hospital staff or hospital social worker.
